Evidenz - Analfistel, Verschluss mit Anal Fistula Plug® (AFP®)

Aus den USA kommend wurde 2006 auch in Deutschland der Anal-Fistula-Plug® eingeführt. Mit Hilfe eines aus Schweinekollagen hergestellten schmalen Zylinders wird der Fistelkanal von innen okkludiert. Auf ein ausgedehnte Entfernung des Fistelgewebes wird im Gegensatz zu anderen Op-Techniken hier verzichtet, was die Operation vereinfacht und nur zu kleinen postoperativen Wunden führt. Nach anfänglich sehr euphorischen Erfolgsberichten mit Heilungsraten von über 85%, konnte in randomisierten Studien nur eine Heilungsrate von 20-30% erreicht werden. Deshalb kommt der Plug heute nur noch in wenigen Fällen als eine von mehreren Optionen bei hohen Fisteln zum Einsatz.
